Susan Houston

Vice President of Older Adult Services

Susan Houston is a recognized leader in the field of services for older adults in San Mateo County, California. For more than twenty years, Susan has provided direct services, managed programs and staff, and successfully developed and implemented new services based on the changing needs of older adults and the community at Peninsula Family Service. Susan has a Bachelor of Arts in both Social Work and Spanish and has completed Masters level courses in Counseling/ Psychology.

Susan was instrumental in the development of a wellness program for low income older adults, county-wide senior peer counseling services targeting the under-served aging population and transition of care programs designed to help older adults transition from an acute care facility to home, focusing on psychosocial support. Susan’s newest developed program, 70Strong, is a service that helps older adults navigate the programs and activities that exist within a specific and diverse health care district in San Mateo County.

Susan has presented at the American Society on Aging conference multiple times with such topics as hospital to home transitions of care, program development and older worker programs and issues. She has also presented at The ARC (Avoid Re-admissions through Collaboration), Partner’s in Care Patient and Family Centered Care Annual Conference and Shining Stars in Health Care. She is also active in a variety of committees including the Sequoia Hospital Community Advisory Committee, The New Beginning Coalition Steering Committee, the San Mateo County Dementia Support Services Advisory Committee and the San Mateo County Continuum of Care.
